  9. Hold the lens between the thumb 
  10. and second finger using the index 
  11. finger to guide the lens.  If the 
  12. lens has an aperture (hole or 
  13. fenestration), it should be on 
  14. the temporal side.  If the optic 
  15. is decentered, the wide portion 
  16. of the haptic should be temporal.   
  17. Have the patient look down and 
  18. pull the upper lid up.
